How can we improve the self service Apps?

The outage page won’t send

I’ve tried several times to report an outage of “light cycles on/off”
As I try to continue after checking that I’ve reported it in the right format, it tells Me “error”
The “light cycles on/off” outage is at
104 N Bryant Ave. Ventnor, NJ 08406 USA
I live on Bryant Ave.
